The Intake Process

At Montrose Hills Therapy | Developmental Education, we provide a personalised and supportive therapy journey tailored to each participant.

 

We understand your goals and needs, then conduct engaging sessions in our serene Therapy Space on the Farm or your natural environment.

 

We prioritise ongoing support and collaboration with families and caregivers. Our process, detailed below, guides you through each step towards working with you.

Express Your Interest

Explore what Montrose Hills Therapy | Developmental Education has to offer. â€‹Complete the 'Expression of Interest' form. The link is available on our homepage and under the 'Express Interest' tab on the website.

 

​The information you provide will be securely stored in our practice management software, splose.

 

A copy of your completed form will be emailed to your nominated address for reference.​

Complementary Meet and Greet

A complimentary meet and greet appointment at the Therapy Space on the Farm in Conway offers a relaxed opportunity for us to get to know each other and for you to explore our space before committing to therapy. It provides a comfortable way to understand what we offer and make an informed connection.

 

Appointment dates and times will be communicated to you.

Please allow 30 minutes for this session.

 

Safety Requirements: For the safety and comfort of all participants, enclosed covered shoes are required.

 

​Note: 'Meet and greet' appointments are contingent on availability. To register your interest, please complete the 'Express Your Interest' form on our homepage at www.montrosehills.com.

Intake Form

Once you're ready to proceed with therapy, you will receive a secure link via SMS or email to complete an Intake Form.

 

This essential document helps us tailor our services to your specific needs and lays the foundation for your therapy journey.

​

Included in the Intake Form is a Preliminary NDIS Service Agreement.

Intake Session

In this approach, the intake session aims to collect comprehensive background information. This involves evaluating the participant's situation and challenges to gain an understanding of their needs and objectives. The session focuses on gathering and planning information, equipping the therapist with a clear insight into existing issues and laying the groundwork for subsequent therapy. Additionally, the therapy goals will be aligned with the participant's NDIS goals to ensure therapy is customised to participant's specific requirements.

This session can be conducted face to face or via telehealth (phone or video calling).

​

* For younger participants, the therapist will meet with parents/carers separately.​

* Older teenagers have the option to participate in the intake session, to assist in determining the most suitable therapy path for their needs.

Therapy Sessions

Therapy sessions are scheduled weekly or fortnightly, lasting 50 minutes, and occur at the same time and day each week or fortnight. Sessions may begin in various parts of the Therapy Space on the Farm, depending on planned activities.

 

Upon arrival, please wait at the brown gate to be greeted by your therapist, as they may be occupied if you arrive early. These sessions aim to create a safe and expressive environment for the participant. For added comfort, parents/carers can join the session, but are encouraged to remain quietly observant, allowing the participant to fully explore their therapeutic journey.

​

For those waiting, shaded or air-conditioned areas are available where you can relax and enjoy the surroundings while remaining within designated spaces for safety. Children not participating in therapy must stay with a responsible adult in the waiting area. Please refrain from engaging with or entering farm animal enclosures, including those with horses, unless accompanied by a therapist.

 

Your therapist will provide a safety briefing upon your initial arrival. Conway Beach is also just a short drive away.

For Home | Community Sessions, (off-site therapy session) your therapist will arrive a few minutes early. These sessions promote free expression in a safe, memorable setting, with support provided to ensure participant comfort. We value the involvement of parents and caregivers, and will discuss their role in skill development at the start of the applicable session.

​

Please note, provider travel will be included in this option.

 

​Please allow 50 minutes for this session.

(Billed: 50min face to face + 10min non face to face + provider travel)
