Alongside Developmental Education and Behaviour Support services, Montrose Hills Therapy provides comprehensive assessments and reports to support understanding, planning, and decision-making.

Assessments provide an evidence base to better understand a person's strengths, functional abilities, support needs, and areas where additional supports may assist.

Assessments may involve direct assessment, observation across environments, consultation with the person, family members, carers, and other support providers, followed by detailed analysis and report writing.

Assessments and reports can support

  • NDIS planning and funding decisions.

  • Plan reviews and change of circumstances.

  • Understanding functional capacity and support needs.

  • Developing personalised recommendations and strategies.

  • Supporting ongoing therapy and capacity-building goals.

Functional Capacity Assessment (FCA)

A comprehensive assessment of a person's functional capacity across the six NDIS domains:

  • Communication.

  • Learning.

  • Mobility.

  • Self-care.

  • Self-management.

  • Social interaction.

FCAs provide insight into a person's strengths, support needs, and areas where capacity building or additional supports may be beneficial.

WHODAS 2.0 (World Health Organization Disability Assessment Schedule)

A standardised assessment that measures functioning and disability across key areas of everyday life, including:

  • Cognition (understanding and communication).

  • Mobility.

  • Self-care.

  • Getting along with others.

  • Life activities.

  • Participation in the community.

WHODAS 2.0 provides information about how a person's disability impacts their daily functioning and participation, helping to inform recommendations, supports, and goal planning.

Functional Behaviour Assessment (FBA)

An in-depth assessment used to understand the reasons behind behaviours of concern.

An FBA explores patterns, triggers, communication, environmental factors, and the purpose a behaviour may serve. Findings help guide the development of effective, person-centred Behaviour Support Plans.

Sensory Profile 2

A standardised assessment exploring sensory processing patterns and how they may impact everyday participation, learning, regulation, and daily activities.

Vineland-3

A standardised assessment measuring adaptive behaviour across areas including communication, daily living skills, socialisation, and motor skills.

ABAS-3

The Adaptive Behavior Assessment System, measuring adaptive skills related to everyday functioning and independence.

PEDI-CAT

The Pediatric Evaluation of Disability Inventory, assessing functional abilities in children and young people.

Other standardised assessments may also be available depending on individual needs and goals.

Assessments and reports can be requested independently or completed as part of an ongoing Developmental Education or Behaviour Support service.
